Administrative Reporting Requirements

Requirement

Sec 1021 Gen. Provision:
Every business shall register with the Regional Labor Office having jurisdiction to form part of databank of all covered establishments

Sec 1023: Period of Registration:
a. For new business within 30 days before operation

Sec 1024: Registration is Free of Charge

Submit to DOLE Regional Office the accomplished Registration of Business Form together with the following documents:

  • Business permit
  • SEC or DTI Registration
  • Location map

Frequency

ONCE and valid for the lifetime of the establishment:

Re-registration as if it were a new establishment is required if:

  1. Change in business name
  2. Change in location
  3. Change in ownership or
  4. Re-opening after previous closing

Requirement

Rule 1041: General Requirements

A Health and safety committee shall be organized within 1 month from date of opening and every January of following year.

Rule 1042: Types and composition of Health and safety committee shall be based on Number of workers

Submit to DOLE Regional office having jurisdiction the accomplished Registration of Health and Safety Committee Organization From together with the following documents:

  • Safety and Health Policy

One month after organization or re-organization of the committee. (February)

  • Minutes of Health and Safety Committee Meeting
  • Safety Performance (No. of Disabling and Non-disabling accidents and Man-hours Worked)

Requirement

Rule 1053.01
Accidents resulting in death and permanent total disability

Rule 1053.02
Dangerous occurrence which may or may not cause serious bodily harm to workers or serious damage to premises

The following are dangerous occurrence:

  • Explosion of boilers and pressure vessels
  • Bursting of grinding wheel (mechanical)
  • Collapse or overturning of crane, derrick, winch or hoist used for lifting persons or goods
  • Explosion or fire causing injuries to people or damage to property

Within 24 hours after occurrence
